Tam High Bike Team FORMS CHECK-LIST

 THE FOLLOWING ARE REQUIRED FOR ANY PARTICIPATION.

Absolutely no participation without the following 5 forms completed.

1. 2007/2008 Tam High Mountain Bike Team Waiver - need a new one for this year for returning athletes as well.

2. Tamalpais Union High School District Student Athlete Clearance Form - need a new one for this year for returning athletes as well.

3. Athlete Code of Conduct - need a new one for returning athletes as well.

4. Ride Rules Form - need a new one for this year for returning athletes as well.

5. Tamarancho Pass - good for 4 years in High School (Mailed directly with payment to address on form--not to Coach.)

All of the above must be completed before athletes are allowed to participate in any team activities.

 THE FOLLOWING ARE REQUIRED FOR RACING. (these forms below are being changed for the 2007 -2008 Season - Do not complete them)

USA Cycling Membership and License Application - (include check for $30 to USA Cycling--Send to address on form, not to Coach--do this by January 1, 2007) do this once per year, or officials will turn you away at the race.   Mandatory for racing!!!

NorCal League Registration Form. From this Website.  Mandatory for racing!!!

or get it from the League Website at NorCal League Registration Form by completing the anonymous online survey - to be completed by high school students only.

USA Cycling Rider/Event Event Release Form (Must be signed by parents and athlete for each race--grab this off the Web site here before each race because parents may not be at the races to sign the required release.)  Mandatory for each race!!!
